Yet one of the most common things we hear at Bossplayah Haven is: “I didn't know I qualified.”

Millions of Washington state residents are eligible for free or very low-cost health coverage and simply don't know it. The paperwork feels overwhelming. The system feels designed for people who already have everything together. And when you're in survival mode, a doctor's appointment can feel like the lowest priority — until it becomes an emergency.

This guide is for you. Whether you need medical care today, are navigating insurance for the first time, or are supporting someone you love through a hard season, this resource breaks down every option available to low-income individuals and families in Washington state. Apple Health (Medicaid) — Washington State's Lifeline

Apple Health is Washington state's Medicaid program, and it is one of the most expansive in the country. If your income is low or you're going through a crisis, there is a very good chance you qualify — and the coverage is comprehensive.

Who Qualifies?

Apple Health uses income thresholds based on the Federal Poverty Level (FPL). As of 2026:

  • Adults (19–64): Qualify at or below 138% FPL — approximately $20,783/year for a single adult or $35,632 for a family of three
  • Pregnant people: Qualify at up to 193% FPL, with coverage beginning the day you apply
  • Children (Apple Health for Kids / CHIP): Covered up to 312% FPL — meaning most children in Washington state qualify, even if parents do not
  • People with disabilities or in long-term care: Covered under separate Apple Health programs with different income rules

You do not need to be a citizen to apply. Immigrant residents, including undocumented individuals, may qualify for emergency Medicaid or the Apple Health Alien Emergency Medical program.

  • In person: Visit any DSHS Community Services Office — find yours at dshs.wa.gov/office-locations
  • Through a navigator: Free, trained community navigators can help you apply at no cost — search for help at wahealthplanfinder.org

Enrollment is open year-round — there is no limited enrollment window for Apple Health. If you lose coverage, experience a qualifying life event (job change, DV situation, leaving incarceration), or have never applied, you can enroll today.

What's Covered?

Apple Health covers a wide range of services with little to no cost to you:

  • Doctor and specialist visits — primary care, OB/GYN, pediatrics, urgent care
  • Prescription medications — most drugs covered at low or no cost
  • Mental health services — therapy, psychiatric care, crisis intervention
  • Substance use disorder (SUD) treatment — inpatient, outpatient, medication-assisted treatment (MAT)
  • Dental care — adult dental coverage expanded significantly in 2023 (see Dental & Vision section below)
  • Vision care — annual exams and eyeglasses for adults
  • Preventive care — vaccinations, screenings, annual physicals

Apple Health for Workers is available for people with moderate income who are employed but whose employer doesn't offer affordable coverage.

Special enrollment is available for people leaving incarceration, exiting a domestic violence (DV) situation, or experiencing other major life disruptions — coverage can often begin immediately.

Community Health Centers & Free Clinics

Even if you don't have insurance, you can still receive care. Washington state has a robust network of Federally Qualified Health Centers (FQHCs) — community clinics that are required by federal law to serve everyone, regardless of ability to pay.

How Sliding Scale Fees Work

FQHCs use a sliding-scale fee structure based on your household income and size. At the lowest income levels, your cost is often $0. You tell them what you earn, they set your fee. No insurance is required, and no one is turned away.

Free Clinics

  • Union Gospel Mission clinics (Seattle and Spokane) offer free primary care, dental, and behavioral health services, with no appointment required at some locations
  • Catholic Charities health access programs connect low-income residents to coverage navigation and care coordination across Western Washington

Mental Health & Substance Use Coverage

Mental health care and addiction treatment are not optional extras — they are core medical needs. Washington state recognizes this, and Apple Health covers both at full parity with physical health care.

  • Individual and group therapy
  • Psychiatric evaluation and medication management
  • Inpatient psychiatric hospitalization
  • Outpatient substance use disorder (SUD) treatment
  • Residential treatment programs
  • Medication-Assisted Treatment (MAT) — including buprenorphine, methadone, and naltrexone for opioid use disorder
  • Peer support specialist services

Behavioral Health Organizations (BHOs) by County

Washington state contracts with regional Behavioral Health Administrative Service Organizations (BH-ASOs) to manage mental health and SUD services. Your county determines which organization coordinates your care. King County uses King County Behavioral Health and Recovery Division (call 206-263-9100). Other counties include Thurston Mason BHO, Optum (Spokane), Beacon Health (Pierce), and more. Your Apple Health plan can connect you directly.

Dental & Vision Care

Dental and vision care are often treated as afterthoughts — but they affect nutrition, employment, mental health, and overall wellbeing. Washington state has made major strides in covering these services.

Adult Dental Under Apple Health

Prior to 2023, adult dental coverage under Apple Health was limited. The 2023 adult dental expansion significantly broadened coverage to include:

  • Routine exams and cleanings (twice per year)
  • X-rays and diagnostic services
  • Fillings and restorations
  • Extractions
  • Dentures and partial dentures
  • Preventive treatments (sealants, fluoride varnish)

Orthodontics and some specialty services may require prior authorization. Confirm coverage details with your Apple Health managed care plan.

Dental Resources for Specific Populations

  • SEAWHIM (Seasonal/Migrant Health): Migrant and seasonal agricultural workers in Washington state may qualify for specialized dental programs through YVFWC and Sea Mar
  • Community dental clinics: Sea Mar, NeighborCare Health, and Dental Lifeline Network Washington provide low-cost and free dental care
  • Dental Lifeline Network WA: dentallifeline.org/washington — serves adults with disabilities, the elderly, and those who are medically fragile

Vision Coverage Under Apple Health

Apple Health covers annual eye exams and one pair of eyeglasses per year for adults. If you wear contacts, coverage may vary by plan — ask your Apple Health plan directly.

Domestic Violence Survivors

Healthcare settings can be both a lifeline and a risk point for DV survivors. You have rights:

  • VAWA (Violence Against Women Act) protections mean you cannot be denied Apple Health or penalized in coverage due to your status as a DV survivor
  • Confidentiality in healthcare settings — you have the right to have a private conversation with your provider without your abuser present, and to receive mail and messages at a safe address
  • Safety planning with providers — trained DV-informed healthcare providers can document injuries, connect you to advocates, and support safety planning
  • WA State Reproductive Health Protections — Washington law protects your right to reproductive healthcare regardless of your partner's wishes or your immigration status

People in Recovery

  • Medication-Assisted Treatment (MAT) — buprenorphine, Suboxone, methadone, and naltrexone are fully covered by Apple Health with no prior authorization required in most cases
  • Opioid Settlement Fund clinics — Washington state's portion of the national opioid settlement is funding new community treatment sites across the state. Your county's behavioral health line can connect you to new resources as they open.
  • Peer support specialists are covered under Apple Health — peers with lived experience in recovery can help you navigate treatment, housing, and the healthcare system

Single Parents

  • WIC (Women, Infants, and Children program) — free food, formula, and nutrition support for pregnant people, new parents, and children under 5. Apply at your local DSHS office or through doh.wa.gov/wic
  • Postpartum care — Apple Health covers 12 months of postpartum coverage for all new parents, including mental health support for postpartum depression and anxiety
  • Family planning — Apple Health covers contraception, STI testing, and reproductive health services
  • Pediatric care — Apple Health for Kids (CHIP) covers children up to 19 at or below 312% FPL, including dental, vision, immunizations, and specialist care

People Experiencing Homelessness

  • Health Care for the Homeless (HCH) programs — FQHCs in Washington state operate HCH programs with mobile units, drop-in clinics, and street outreach specifically for people without stable housing
  • King County Public Health street outreach — public health nurses and community health workers conduct outreach in encampments, shelters, and transitional housing
  • Mobile health clinics — organizations like Country Doctor Community Clinic and International Community Health Services operate mobile units across King County
  • You don't need a permanent address to apply for Apple Health — you can use a shelter address, a service provider's address, or a P.O. box

Returning Citizens (People Leaving Incarceration)

Washington state is a national leader in re-entry health coverage. Under HB 1259, Washington was among the first states to begin restoring Medicaid coverage before someone leaves incarceration, so coverage is active on the day of release. This means:

  • You can see a doctor, fill prescriptions, and access mental health care the day you're released
  • Reentry case managers at the jail or prison can help enroll you before release
  • If this didn't happen, apply immediately at wahealthplanfinder.org — you may qualify for same-day coverage

Prescription Assistance

The cost of medication should never prevent someone from getting treatment.

Through Apple Health

Apple Health covers most prescription medications at low or no cost. Generic drugs are typically free or $1–$3. If your medication isn't on the formulary, your doctor can request a prior authorization. Don't skip doses — call your Apple Health plan first.

For the Uninsured or Underinsured

If you don't yet have Apple Health or are in a coverage gap, these programs can help:

  • NeedyMeds.org — national database of drug manufacturer assistance programs, free clinics, and discount cards
  • RxAssist (rxassist.org) — guides to Patient Assistance Programs (PAPs) from pharmaceutical manufacturers; many offer free or deeply discounted brand-name drugs
  • GoodRx (goodrx.com) — free discount card that can reduce drug costs by 70–80% at most WA pharmacies
  • Washington Prescription Drug Program — the state's own discount program; visit rx.wa.gov for a free discount card with no eligibility requirements

Patient Assistance Programs (PAPs) are run directly by drug manufacturers for people who can't afford their medications. Your doctor, pharmacist, or FQHC navigator can help you apply.
